How to Participate in the FAIRWINDS Credit Union Member Satisfaction Survey

What You Will Need:

  • A PC/ a mobile gadget.
  • A stable internet connection.
  • A valid service center receipt.

Instructions:

  1. Using your browser, navigate to www.fairwinds.org/survey.
  2. Enter a 4 to 6-digit Teller-ID. You will find this on your service center receipt. It is labeled TLR and it is below the date.
  3. Select the location of your transaction; either drive-thru or counter.
  4. Rate the nine categories provided on a rating scale. In some cases, you rate with either a yes or a no while some categories involve rating with one of the following: Excellent, Very Good, Average, Below Average, Poor, or N.A.
  5. Enter your personal information: name, mobile number, email address, and comments.
  6. Click ‘submit’ button.
