  • Three to five minutes of time.
  • To be fluent in English.

  1. Visit www.hy-veesurvey.com.
  2. To start the survey, enter in the following information from your latest Hy-Vee receipt: the date you visited, if you or any of your immediate family is employed by Hy-Vee, the 16-digit number from your receipt and the 4-digit store number.
  3. Answer the rest of the questions regarding your most recent visit to Hy-Vee as accurately and as candidly as possible.
  4. Enter in your personal feedback if you have any thoughts or opinions on how Hy-Vee can serve you better as a customer.
  5. Enter any additional comments you want to pass on to Hy-Vee about your shopping experience.
  6. Upon completion of the survey, you have the opportunity to enter into a monthly drawing for a Hy-Vee gift card worth $500.
